Who can refer into Active Herts?

We encourage referrals from healthcare professionals including GPs, Nurses, Healthcare Assistants, Physios and Mental Health Workers. Community groups and organisations who have contact with service users can all sign up as referrers.

We would like to work with a range of organisations that have conversations and interactions with inactive people on a regular basis, where a conversation around the benefits of physical activity might come up in discussion.

What’s my role as a referrer?

As a healthcare professional you are well placed to raise the topic of physical activity and its benefits to individual’s health. 80% of our referrals are received from Healthcare Professionals and we know people are more likely to act on the recommendation from a professional.

Who should I refer?

Before making a referral, please make sure the client can answer ‘YES’ to following inclusion criteria to make sure they are eligible: –

  1. Is the individual currently inactive, doing less than 30 minutes of physical activity that raises their breathing each week?
  2. Are they over the age of 16?
  3. Do they live in the boroughs of Broxbourne, Hertsmere, Stevenage or Watford?

Can I refer people who aren’t inactive?

Yes. However if people are not inactive they will only be offered information about types and locations of activities in their area and not the full support package.

How do I refer someone? 

Providing the individual fits the eligibility criteria they can be referred directly to their local Get Active Specialist. Please see ‘how to make a referral
